GPs and other referrers can refer a client to Jetlyn in any of the following ways:

• Provide the person with Jetlyn’s contact details and they can make contact themselves.
• Email or fax a completed MHTP or referral letter, please ensure the person’s contact details are correct. Please note the client will need to contact Jetlyn to arrange an initial appointment.
• Contact Jetlyn directly to discuss your patient.

Jetlyn is practising at the Capital Clinical Psychology Consulting Rooms, located at Lvl 1, 5 Sargood Street, O’Connor, ACT.  Further details about the location and parking information can be found on the contact page.

Jetlyn also offers telehealth appointments via Coviu (secure telehealth software).

After you book your first appointment you will be emailed a link to fill in the online intake form. The online intake form will ask for personal details, payment details and there will be a consent form to read through and sign. Jetlyn will go through this information with you at your first appointment and answer any questions you might have.
